RELATED FORMS
The following forms are to be utilized as you work through the Performance Excellence process. Your unit or college may have additional forms that are required as well.
Step 1: Performance Planning Meeting
- Performance Planning Form (PDF) This form must be completed during the Performance Planning meeting and signed by you and your employee. Follow your MAU or college's specific process for routing or email a copy of the signed Performance Planning form to MSU HR by the due date.

Mid-Cycle Check-In (PDF)
This worksheet supports meaningful conversations and preparation for discussions between employees and supervisors. This worksheet is for internal use only and does not need to be sent to MSU Human Resources. -
Probationary Form (PDF)
For employees new to their role who have a probationary period less than 12 months (CTU, 1585, etc), supervisors should complete a Probationary Review at the end of the required probationary period to indicate a successful/unsuccessful completion of the probationary period. -
Interm Review Form (PDF)
For employees new to their role that have a 12-month probationary period (APA, APSA), supervisors should complete an Interim Review at least halfway through the probationary period (approximately 6 months). These employees will then receive their Annual Performance Review at the one year mark, which will mark the end of the probationary period (if successful). No additional Probationary Review form is required.
Step 3: Annual Performance Review
- Annual Performance Review Form (PDF) Complete this form during the Annual Performance Review with your employee. You must choose and give a rationale for an overall performance level. Your MAU or college may have additional forms and documentation requirements. Follow your MAU or college's specific routing process or email a copy of the signed Annual Performance Review form to MSU HR by the due date.
- Self-Review Worksheet (PDF) Consider asking your employee to complete this self-review worksheet prior to the Annual Performance Review to help them prepare. Consider also having the employee send the self-review worksheet to you in advance as this is a great opportunity for the employee to share their accomplishments to help inform the annual review.

Performance Improvement Plan (PDF)
If an employee receives a Does Not Meet Expectations performance level they must undergo a Performance Improvement Plan (PIP). Please consult with your local HR Representative and MSU Employee and Labor Relations prior to the annual review for additional guidance and support.
