Completing the form for Faculty & Academic Staff Leave Related to Pregnancy, Childbirth and/or Recovery; and Parental Leave

The following scenarios are provided as examples of how to fill in the Leave of Absence and Return form for Medical/Sickness (Short-Term Disability) leave related to pregnancy, childbirth and/or recovery, and/or Parental leave under various circumstances; they do not address all possible situations.

Birth-mother provides medical certification indicating a projected delivery date of May 1, 2011 and requests that her leave begin 2 weeks before the projected delivery.

  • Parental leave 2 weeks: 4/18/2011 - 5/01/2011
  • Maternity leave 6 weeks: 5/2/2011 - 6/12/2011
  • Parental leave 4 weeks: 6/13/2011 - 7/10/2011
  • Leave Return: 7/11/2011
  • Pay Dates of Leave:  4/18/2011 - 7/10/2011

Birth-mother provides medical certification specifying that she is unable to work from March 15, 2011 through June 15, 2011 due to birth of child expected May 1, 2011.  She requests that the leave begin March 15, 2011.

  • Medical/Sickness (Short Term Disability): 3/15/2011 - 6/12/2011
  • Parental leave 6 weeks: 6/13/2011 - 7/10/2011
  • Leave Return: 7/11/2011
  • Pay Dates of Leave:  3/15/2011 - 7/10/2011

The birth-mother in #2 above is appointed on an academic year (AY) basis and we would exclude the non-duty period (5/16/2011 – 8/15/2011) from her leave.

  • Medical/Sickness: 3/15/2011 – 5/15/2011
  • Leave Return: 5/16/2011
    Parental leave 6 weeks during duty days: 8/16/2011 - 9/26/2011
  • Leave Return: 9/27/2011
  • Pay Dates of Leave:  3/15/2011 – 5/15/2011 and 8/16/2011 - 9/26/2011

Adoptive parent appointed on a annual year (AN) basis provides evidence of date of adoption as April 1, 2011 and evidence of age of child as one year old, and requests that the leave begin April 25, 2011.

  • Parental leave runs 6 weeks: 4/25/2011 – 5/29/2011
  • Leave Return: 5/30/2011
  • Pay Dates of Leave:  4/25/2011 - 5/29/2011

The adoptive parent in #4 above is appointed on an academic year (AY) basis and requests that the parental leave clock stop during the non-duty days.

  • Parental leave 3 weeks: 4/25/2011 - 5/15/2011
  • Leave Return: 5/16/2011
  • Parental leave 3 weeks: 8/16/2011 - 9/5/2011
  • Leave Return: 9/6/2011
  • Pay Dates of Leave:  4/25/2011 - 5/15/2011 and 8/16/2011 - 9/5/2011

Adoptive parent provides evidence of date of adoption as April 1, 2011 and evidence of age of child as three years old, and requests that the leave begin March 28, 2011 and that she/he work 50% time for a 12-week period while on parental leave.  The unit administrator agrees that the intermittent leave is in the best interests of the unit.

  • Parental leave is for 50% time for the 12-week period of 3/28/2011 - 6/19/2011

Note:  The details of the intermittent arrangement may be shown on the Leave of Absence form on the comments lines, or they may be kept in the department only and not appear on the form.  However, the dates on the leave form should reflect a six-week leave period starting with the first day of the intermittent leave.

  • Pay Dates of Leave:  3/28/2011 -  5/8/2011 (not the actual 12-week period)
  • Parental leave: 3/28/2011 – 5/8/2011
  • Leave return: 5/9/2011

Comments:  At the department's discretion the 50% time for the 12-week period of 3/28/2011 to 6/19/2011 can be referenced on the Comment section of the Leave form.

Parent provides evidence of birth of child on March 1, 2011 and Maternity leave is from 3/1/2011 – 4/11/2011.  Parent requests Parental leave in three two-week increments.  4/12/2011 – 4/25/2011, 5/10/2011 – 5/23/2011, and 6/7/2011 – 6/20/2011.   The unit administrator agrees that the intermittent leave is in the best interests of the unit.

  • Maternity leave: 3/1/2011 – 4/11/2011
  • Parental leave 2 weeks: 4/12/2011 – 4/25/2011
  • Leave return: 4/26/2011
  • Parental leave 2 weeks: 5/10/2011 – 5/23/2011
  • Leave return: 5/24/2011
  • Parental leave 2 weeks: 6/7/2011-6/20/2011
  • Leave return: 6/21/2011

Birth-mother provides medical certification indicating a projected delivery date of March 1, 2011 and requests that her leave begin March 1, 2011.

The other parent is also an MSU faculty/academic staff member and provides a copy of the projected delivery date document and requests a 3 week leave beginning March 1, 2011.

Mother's Leave of Absence:

  • Maternity leave 6 weeks: 3/1/2011 - 4/11/2011
  • Parental leave 3 weeks: 4/12/2011 - 5/2/2011
  • Pay Dates of Leave:  3/1/2011 - 5/2/2011

The other parent's Leave of Absence:

  • Parental leave 3 weeks: 3/1/2011 - 3/21/2011
  • Pay Dates of Leave:  3/1/2011 - 3/21/2011
