QUALIFYING LIFE EVENT: MARITAL STATUS

Marital Status Checklist:

Marriage

Enrolling New Dependent(s) in Benefit Plans:

  • Submit copy of marriage license or certificate to MSU Human Resources within 30 days of marriage, and
  • Complete required forms in the EBS Portal within 30 days of marriage.
  • Once forms are processed, you must enroll children in benefits through the EBS Portal.

Other items you may want to consider as a result of your marriage:

  • Change your Beneficiary Designation for any life/accident insurance policies you may have.
  • Contact your retirement  investment sponsor(s) directly to make beneficiary change.
  • Make address or phone number changes in the EBS Portal.
  • If you are changing your name give a copy of your new Social Security card to your department or directly to Human Resources, 1407 S. Harrison Rd., Suite 110.
    • To obtain a new Social Security card take appropriate documentation (marriage license, court documents) to the nearest Social Security Office.
  • Contact the MSU WorkLife Office or  Employee Assistance Program (EAP) for information on services that are available for helping your child(ren) adjust to remarriage.

Legal Separation or Divorce

Removing Dependent(s)/Enrolling in Benefits:

  • Submit a copy of your official legal/court documents to MSU Human Resources within 30 days of the legal separation or divorce being finalized.
  • If applicable complete a Loss of Previous Coverage form and submit it to the MSU Human Resources within 30 days of the legal separation or divorce being finalized.

Other items you may want to consider as a result of your legal separation or divorce:

  • Change your Beneficiary Designation for any life/accident insurance policies you may have.
  • Contact your retirement  investment sponsor(s) directly to make beneficiary change.
  • Make address or phone number changes in the EBS Portal.
  • If you are changing your name give a copy of your new Social Security card to your department or directly to Human Resources, 1407 S. Harrison Rd., Suite 110.
