Graduate assistant cobra faqs

You will receive a COBRA enrollment form that will be sent out to you from WageWorks after your insurance coverage has ended. You will need to complete and sign the enrollment form and return it to WageWorks. Once they have received your enrollment information, they will send it to the insurance company who will process the enrollment.

You must complete enrollment within 60 days after the date of the notice.

If you lose coverage on 02/15, your coverage will begin on 02/16. If you lose coverage on 08/15, your coverage will begin on 08/16.

Initial Enrollment: Your first payment will be prorated.

Student- $293.08
Spouse-$293.08
Child- $293.08
2 or more children- $586.16

No, you will not receive a new BCN ID number and insurance card. Your BCN ID will remain the same.
Payments are due within 45 days after your election is made. You must make your first payment no later than 45 days after the date of your election. (This is the date the Election Notice is postmarked). You will then be billed on a monthly basis. You may pay by check.
