1095-C Tax Form

Under the healthcare reform law, most Americans are now required to have health insurance or pay a penalty on their income taxes. Full-time employees, and employees, retirees and COBRA participants enrolled in a Michigan State University medical plan, will receive a Form 1095-C.

Here’s why you need Form 1095-C:

  • If you enrolled in Blue Cross Blue Shield (including the Consumer Driven Health Plan) or Blue Care Network coverage in 2016, the form provides the information needed to report on your income tax return that you (and your spouse and dependents, if applicable) had qualifying health coverage for some or all months during the year.
  • If you were offered access to an MSU health plan, whether or not you actually enrolled, the form will indicate that you had access to health coverage.

Human Resources will send the Form 1095-C around March 2, 2017 to your home address on file. You do not need to attach it to your income tax return – just use it for reference.
