I-9 Form Completion by Notary or Other Agent Acting on Behalf of MSU

Form I-9, Employment Eligibility Verification

Information and Instructions for Notaries or Other Agents Acting on Behalf of Michigan State University

Legal Requirement

The Immigration Reform and Control Act requires all U.S. employers to verify the employment eligibility and identity of all employees hired to work in the United States after November 6, 1986. To implement this law, employers are required to complete the Employment Eligibility Verification form (Form I-9) for all employees, including U.S. citizens.

Instructions

Michigan State University (MSU) requests that you act as an agent on behalf of the University to complete Section II, Employer Review and Verification, of the I-9 form. We ask that you perform this role in order for the I-9 to be completed within the time limits stipulated by the law.

To complete this section (for U.S. citizens or permanent residents only) the MSU employee must provide original documents as identified on the I-9 form (one document from column A, or one each from columns B and C, which are listed on the back of the I-9 form).

  • You must accept any document(s) presented which reasonably appears on its face value to be genuine and related to the person presenting it.
  • You may not specify which document(s) are presented.
  • Do not use whiteout to correct an error. If a mistake occurs, cross out the error, initial and date it, and insert the correct information.
  • Record the following information in Section II for List A or Lists B and C:
    • Document title
    • Issuing authority
    • Document number
    • Expiration date, if any
  • Complete the Certification section:
    • Leave the employment beginning date blank
    • Sign and print your name
    • Under "business or organization name," fill in "for MSU"
    • Fill in your (notary or other agent) title and complete address
      (including zip code)
    • Fill in date form is signed
    • Do not complete Section 3
  • Attach a copy of the document(s) you reviewed to the I-9 form; return document(s) originals to the MSU employee.
