Has my dental benefits administrator changed? 
Yes. Delta Dental of Michigan will be your new dental benefits administrator. You will be covered under Delta Dental PPO (Standard).

What is Delta Dental PPO (Standard)? 
Delta Dental PPO (Standard) is Delta Dental’s national preferred provider organization program that ensures extensive network access, guarantees no balance billing when you use participating providers and enforces processing policies that will save you money. In order to receive dental benefits, you may only seek treatment from a Delta Dental PPO provider. If you visit a dentist who is not Delta Dental PPO participating, you will be responsible for the entire cost of treatment.

How can I enroll in the Delta Dental Plan?
To learn more or enroll, you can call Delta Dental's customer service department at 1-800-971-4108 for premium or enrollment inquiries. You may also enroll online by choosing the appropriate link below:

Once I have enrolled, can I access information about my dental plan online? 
You can use Delta Dental's online consumer toolkit at www.deltadentalmi.com to access your own benefit, claims and eligibility information 24 hours a day, seven days a week. You can also use this Toolkit to search their dentist directories, print customized ID cards and claim forms, and read oral health tips.

How can I find a participating dentist?
To find a Delta Dental PPO participating dentist near you, call their customer service department toll-free at 1-800-524-0149. Delta Dental’s Automated Service Inquiry system is available 24 hours a day, seven days a week, and can provide you with the names of participating dentists. You also can access their online provider directories at www.deltadentalmi.com.

What are the advantages of choosing a Delta Dental PPO dentist?
This plan will only pay benefits to a Delta Dental PPO dentist. Delta Dental pays PPO dentists based on their submitted fee or the amount in Delta Dental’s PPO dentist schedule, whichever is less. Delta Dental PPO dentists are prohibited from charging you the difference between their submitted fee and the amount that Delta Dental allows. You will be responsible only for your copayments and deductible, if any, for covered services. Delta Dental PPO providers also are required to fill out and file claim forms for you, and Delta Dental will send payment directly to the provider – you don’t have to pay up front and wait to be reimbursed.  

What if I go to a non-PPO dentist?
If you go to a non-PPO dentist and that dentist participates in another Delta Dental program called Delta Dental Premier, you will be responsible for payment of dental services up to Delta Dental’s Maximum Approved Fee. If you go to a nonparticipating dentist (that is, a dentist who does not participate in Delta Dental PPO or Delta Dental Premier), you will be responsible for payment up to the dentist’s submitted fee. You will have to pay the dentist at the time of your appointment, and you may need to submit your own claims.

Can I request that Delta Dental attempt to recruit my dentist? 
Yes. If your current dentist does not participate in Delta Dental PPO, you can recommend him or her for membership by completing an online referral form. You also may call Delta Dental's customer service department at 1-800-524-0149 to refer your dentist. Please note that it is each dentist’s decision whether or not to join, and each dentist accepted by Delta Dental must first meet the qualifying criteria established for each program.

Do I need to tell my dentist that my coverage has changed?
Yes, it would be helpful if you told your dentist that you have Delta Dental PPO (Standard) coverage through Delta Dental of Michigan. For services rendered after you have enrolled in Delta Dental, remind your dentist to submit claims to Delta Dental, P.O. Box 9085, Farmington Hills, MI 48333-9085.

Where should claims for services rendered prior to enrolling in Dental Dental PPO (Standard) be sent?
For any services rendered prior to being enrolled with Dental Dental, your claims must be submitted to your prior dental benefits administrator for reimbursement.
