Actions Processed by Memorandum
- Name change: To change a name in the middle of an appointment period, send a copy of the social security card showing the new name with a request for a name change, to Academic Human Resources, Room 312 Administration. The name used in the University computer records must match the social security card. However, the space allowed for a name in the faculty computer records system is limited to 30 characters including punctuation and spaces.
To change a name at the time an Appointment/Reappointment is processed, just use the new name on the Appointment/Reappointment form and attach a copy of the new social security card.
- Racial/ethnic identification correction: Send a memo to Academic Human Resources, Room 312 Administration, stating the correct ethnic id. The request must be signed by the faculty/academic staff member.
- Person with disability/veteran status: New faculty and academic staff are surveyed in the year of hire to determine disability and veteran status, and are asked to update the information every three years thereafter. If an individual wishes to update their status at some other time they can call Academic Human Resources (5-1526) or email dionise@msu.edu to request a blank form letter which they can fill out to indicate disability and/or veteran status.
