The Sabbatical Leave Information (Faculty/Academic Staff) form is submitted as an attachment to the Leave of Absence form for Sabbatical leaves. Also see Sabbatical Leave Implementation Information.
Whenever possible the AHRforms web application (pre-populated forms application) must be used to produce this form. This form is automatically produced in that application when a sabbatical leave is indicated on the Leave of Absence form. On the rare occasions when it is impossible to use the AHRforms web application (e.g., because the server is down and deadline is imminent), use the fill-in Sabbatical Leave Information (Faculty/Academic Staff) form.

Name: As recorded on the Social Security card, except type last name first with a comma after and then the rest of the name.
Special supplemental income arrangements during leave: A response must be given; write "none" if there are no special supplemental income arrangements.
Faculty signature: Signature of the individual who is going on sabbatical leave.
Chairperson/Director signature: The signature of Chairperson/School Director must be present.
Dean signature: The signature of the dean must be present.
Routing: Follows the routing of the Leave of Absence form to which it is attached.
