Information about the INS-I9 form
The INS I-9, Employment Eligibility Verification form must be filled out for every first-time appointee into a faculty/academic staff position. For U.S. citizens and permanent residents, one I-9 form will be sufficient for the duration of their academic employment with MSU, although new copies of the completed form may be required as an attachment to further appointment actions. For non-resident foreign nationals, the employment eligibility is granted on a temporary basis, and thus a new I-9 form must be filled out any time their proposed employment at MSU extends beyond the employment eligibility end date of the most recent I-9 form.
This form may be photocopied.
Circle "Faculty" at the top of the form for both faculty and academic staff.
Section 1. Employee Information and Verification:
After the employee completes Section 1, check that the information is legible, complete (including date of birth and the "I attest under penalty of perjury..." section), and that the employee has signed and dated it. The name should be printed as recorded on the Social Security card, except last name first with a comma after and then the rest of the name. Section 2. Employer Review and Verification:
For non-resident aliens (i.e., foreign nationals who are not permanent residents) this section must be filled out and signed by the Office of International Students and Scholars (OISS) 353-1720. The MSU appointment for a non-resident alien may not extend beyond the end date of the employment authorization granted by the Immigration and Naturalization Service (INS).
For U.S. citizens and permanent residents this section is completed by an MSU employee who is authorized by the unit to do this (usually the same person who types the personnel forms for the unit). In cases where the prospective employee is not available on campus to complete this form at the time the Appointment form is completed, the prospective employee may be directed to take the form to a notary who will fill out Section 2 and the Certification section on MSU's behalf.
To complete this section ask the faculty/academic member to provide original documents as identified on the I-9 form (one document from column A, or one each from columns B and C which are printed on the back of the I-9 form). Examine the documents in the presence of their owner and complete section 2 with the requested information from the documents, and then return the documents to their owner.
Certification: The person who reviews the documents and fills in Section 2 should sign the form on behalf of MSU. If the form does not already have the words "for MSU" typed at the end of the "Signature of Employer or Authorized Representative" box, print them in after your signature. Routing: Follows the routing of the Appointment or Reappointment form to which it is attached.
