The Academic Hiring Report form documents the hiring process when positions are posted. The form is completed in sections in accordance with the Academic Hiring Procedure. (If a position was not posted, complete the Waiver information on the Appointment form rather than using this form.)

Appointee's Name: As recorded on the Social Security card, except type last name first with a comma after and then the rest of the name. (This is completed after the candidate has accepted the position.)
Date: Typing date.
Position #: If the college assigns the Academic Position number make sure it is typed at the top of each page.
Availability Data: % availability, % utilization, and unit hiring goals are shown on "Table A. Placement Goals for Faculty and Academic Staff" which is part of the "Affirmative Action Plan Update" memo which is sent annually (usually in October) to Deans, with a request that the Deans distribute the information to departments. If you do not have a copy of this table contact your dean's office. Fill in this information for all positions, not just tenure system positions. A group (e.g., women, total minorities, Black, etc.) is "underutilized" if on Table A there is a minus sign before the number in the "% utilization" row for that group.

Part 1: Summary of Applicants...: Double check the numerical summary of applicants Section D, Part 1 for accuracy.
Signatures: Signatures must be the primary chairperson/director's and dean/MAU administrator's name with any other initials below. Each signer must date her/his signature.
