Performance Development Program (PDP)

The Performance Development Program (PDP) is the official University program for support staff performance evaluation and development. 

All support personnel should receive a regular PDP evaluation annually.  Human Resources sends the official PDP Form to the department approximately one month prior to the employee's PDP due date.  Once the PDP discussion has taken place between the supervisor and the employee, the form should be signed by both parties and the original form returned to Human Resource Records for inclusion in the official personnel folder. 

PDP Supplemental Forms

The PDP II Form is an optional form that departments may use to provide additional commentary beyond the space provided on the regular PDP Form. 

The Employee Self Review Worksheet is an optional form that employees are encouraged to use to prepare for the PDP discussion and to reflect on their performance and contributions over the review period. 

The Performance Improvement Plan Form is a required form in cases where an employee 'Does Not Meet Expectations' on the official PDP form.   It must be completed after the official PDP review.  The Performance Improvement Plan requires that there be three subsequent improvement reviews over the 90 day period immediately following the PDP review.  Once the 90 day improvement period is completed, the form should be signed and forwarded to Human Resource Records for inclusion in the official personnel folder. 

The Interim Evaluation Form is used only for APA and APSA employees who are within their 12 month probationary period.  Human Resources generates a pre-printed form and sends it to the unit when an employee has completed 6 months of their probationary period.  The form, summarizing overall job performance, is completed by the employee's supervisor.  If a probationary employee's performance is deemed to be unsatisfactory, such information must be communicated to the employee as soon as practicable in order to give the employee the opportunity to improve his or her performance. 

The Probationary Evaluation Form is used only for CTU and off-campus employees who are within their applicable probationary period.  Human Resources generates a pre-printed form and sends it to the unit when an employee is nearing completion of probation.  The form, summarizing overall job performance, is completed by the employee's supervisor.  If a probationary employee's performance is deemed to be unsatisfactory, such information must be communicated to the employee as soon as practicable in order to give the employee the opportunity to improve his or her performance.
