Accidental Death and Dismemberment (AD&D) Insurance

Policy

The University provides access to an Accidental Death and Dismemberment (AD&D) plan for eligible employees.

Eligibility:


All regular full-time and part-time (50 % time or more) faculty and staff, and members of the Board of Trustees of the University.

Effective date of benefit: 

Coverage will be effective as of the date of employment or appointment to an eligible status provided enrollment occurs within 60 days.  Enrollment beyond 60 days cannot occur until the annual open enrollment period.  Employees not actively at work on that day will become covered on the day they return to work.

Summary of benefits:

  1. The benefit is payable, in whole or in part, in the event of accidental:
    • death,
    • dismemberment, or
    • loss of sight.
  2. Benefit coverage may be selected in varying amounts depending on plan selected (see Schedule of Benefits below).
  3. Coverage may also be selected for eligible dependents (see Schedule of Benefits below).
  4. Spouse/MSUrecognized same-sex domestic partner coverage: 40% of employee coverage (50% if, on the date of the accident, the employee has no dependent children covered).
  5. Child(ren) coverage: 5% of employee coverage (10% if, on the date of the accident, the employee has no spouse/same-sex domestic partner covered).

Premium contribution:

 

The program is entirely funded from employee premiums and rates are subject to change. The employee's monthly contributions are shown above in the Schedule of Benefits chart and in the AD&D brochure available online or at the MSU Human Resources Benefits office.

 

Termination date of benefit:

  1. Coverage will terminate on the last day of the month following the month in which the last deduction is taken.
  2. Employees on unpaid leave of absence, who wish to maintain coverage, must forward the full premium payment to MSU Human Resources Benefits. Coverage allowed to lapse, due to non-payment of premiums, will automatically be reinstated to the coverage in effect prior to the leave. (The employee must return to an eligible status.)
  3. Dependent coverage terminates as above. In addition, certain changes to dependent status will terminate coverage for dependents.

Conversion of coverage:

 

No conversion is possible.


Procedure

Application for enrollment:

Employee:

  1. Enroll within 60 days of initial employment or appointment to an eligible status by accessing the web site at www.hr.msu.edu; click the eHR Services Login link, enter your MSUnetID/Pilot ID and password.
  2. If enrollment does not occur within 60 days of initial employment or appointment to an eligible status it is necessary to wait for an annual open enrollment period.

Changes in coverage or personal information:

Employee:

  1. Change the amount of coverage:
    • may increase or decrease coverage
    • may cancel coverage at any time, or
    • may add eligible dependents.
  2. Change of beneficiary: May change beneficiary at any time by filling out a Beneficiary Designation Form and returning it to MSU Human Resources Benefits.

Filing a claim:

Department:

  1. Notify MSU Human Resources Benefits immediately upon the accidental death, dismemberment or loss of sight of an employee.
  2. Refer questions concerning benefits of deceased employee to MSU Human Resources Benefits.

Employee:

  1. Notify MSU Human Resources Benefits in the event of the accidental death of a covered dependent.
  2. Notify MSU Human Resources Benefits in the event of the accidental dismemberment or loss of sight of self or a covered dependent.
