5.2 Processing An On-Call Appointment
An Appointment form is filled out for those without a previous paid or on-call academic appointment; a Reappointment form is filled out for reappointments. The Appointment/Reappointment form must be completely processed (i.e., be approved by the Dean, and Provost, and be entered into the Academic Human Resources computer system) before Payroll can process payments. Thus it is important to fill out the form and submit it for approval well in advance of the period during which the work will be performed.
Fill out an Appointment form with the following information:
- Name
- Social Security number
- Date (date the form is typed)
- Appointment Dates: Begin/End (The beginning and end date of the period during which the work will be done, up to a maximum of three years. The individual may be reappointed if they continue to provide service beyond the initial period indicated on the appointment form.)
- Appointment Basis (check the "Annual year" box)
- Position/Rank (leave blank if there is a title but no position/rank) If appointing an MSU academic retiree, use the emeritus rank (for example, professor emeritus) rather than a different rank associated with the on-call function.
- Title (leave blank if there is no title) If appointing an MSU academic retiree, use the emeritus title (for example, director emeritus) rather than a different title associated with the on-call function.
- Appointment Status (Check the appropriate category in the first big box, e.g., faculty, specialist, other, etc. Check "On-call" in the second big box.)
- Department/College
- Date of Birth
- Ethnic ID
- Gender
- U.S. Citizen. If No, complete the Type of Visa and Country of Citizenship
- Degree, School, Date
- Any relative employed... If Yes, complete the Name/Relation and Conflict of Interest form information
- Salary (= 0)
- Percent employed (= 0)
To the Appointment form attach a completed INS I-9 Employment Eligibility Verification form, a W-4 form, and a copy of the social security card.
Forward the Appointment plus attachments to the Chairperson. From there the materials are routed successively to the Dean, Academic Human Resources, Provost Office, Academic Human Resources and to Payroll.
