Recipients: All faculty and staff required to complete an affidavit that had not yet gone through online.
Subject: Termination of spousal benefits
Benefits Open Enrollment for the 2007-2008 plan year ended on May 7, 2007. We are writing because we noticed that you currently cover a spouse or other eligible individual (OEI) under your benefits but you did not participate in open enrollment and complete an affidavit to continue that coverage into the 2007-2008 plan year. This means that coverage of your spouse or OEI is scheduled to be terminated effective July 1, 2007. If you intend to end this coverage, you do not need to do anything. Termination will automatically occur on July 1.
But we know that sometimes when employees do not complete an affidavit, it is the result of an oversight or due to a lack of understanding that failing to complete an affidavit every year will result in the termination of spousal or OEI benefits in the next plan year.
Though open enrollment is over, we have not yet shut off your access to the online open enrollment system. If you do not want your current coverage for your spouse or OEI to be terminated effective July 1, 2007, please participate in the online open enrollment by the end of the day on Friday, May 11, 2007.
