According to our 2006 Open Enrollment records, you have selected the Aetna DMO to provide dental coverage for you and your family.
If you live in the Lansing area, there are currently three approved providers for you to select from. They include:

Midwestern Dental
Dr. Hina Shah
If you do not live in the Lansing area, you can determine who the approved providers are in your area by visiting www.aetna.com . You can select your primary care dentist online at www.aetna.com or call Aetna at 1-877-238-6200.
As you have selected a Dental Maintenance Organization (DMO), it will be important for you to remember that after your coverage becomes effective on July 1, 2006, you must seek your dental care ONLY from the dentist you select as your primary care dentist. The dentist you select as your primary care dentist must participate in the Aetna DMO plan.
