How are dependents added to or dropped from my dental plan?
You may add or delete dependents during the plan year by enrolling online through our Benefits Enrollment/Changes online enrollment system for the following qualifying life events:
Changes must be made within 31 days of the event for:
- Marriage or eligible MSU recognized same-sex domestic partnership
- Birth of a new baby, adoptions or legal guardianship
- Involuntary loss of coverage through a non-MSU plan
- Divorce
Changes must be made within 60 days of the event for:
- Remove dependent(s)
- Change in employment percent (i.e. full-time to part-time, part-time to full-time)
- New hire
Documentation such as birth certificates, marriage certificate, MSU recognized same-sex domestic partner agreements, divorce decree must be received within 7 days of making the change online.
My dentist is not located in Michigan, how do I file a claim?
Aetna
Aetna is a dental maintenance organization (DMO), therefore you must see a primary care dentist in their provider network. Since Aetna is a DMO, there are no claim forms to be submitted.
Delta Dental
If your dentist is not located in Michigan or if your dentist cannot provide you with a claim form, you may obtain one from MSU Human Resources Benefits or Delta Dental Plan of Michigan. Claims, adjustment requests and completed information requests should be mailed to Delta Dental, P.O. Box 9085, Farmington Hills, MI 48333-9085.
I am the policyholder, am I eligible for the orthodontia benefit?
Aetna offers orthodontic benefits to children and adults. Please see the Aetna Brochure for copay amounts.
Delta Dental Plan pays a lifetime maximum of $600 for orthodontic benefits for you and each of your eligible dependents, provided you or your eligible dependent is under age 19.
I missed this year's open enrollment period. How can I change dental plan carriers? Can I still add my spouse/dependents to my policy?
- To change Dental carriers you must wait for the next open enrollment period.
- If dependents are newly acquired (marriage, birth, adoption, etc.), you may add them to health and/or dental plans within 31 days through our Benefits Enrollment/Changes online enrollment system. Otherwise you must wait for the next open enrollment period.
Who is eligible as a dependent on my dental policies?
Eligible dependents include your legal spouse/MSU recognized same-sex domestic partner and unmarried children living in your household (unless a full-time student at an accredited college or university), who are dependent on you for support and maintenance according to IRS definition, except in divorce cases.
Stepchildren and legally-adopted children are eligible dependents.
Parents or other relatives over age 25 are eligible to be covered under your health plan with a self-purchased sponsored dependent rider, if living in your household (unless a full-time student at an accredited college or university) and primarily dependent on you for support and maintenance according to IRS definition. They are not eligible for coverage under the Dental, Employee-Paid Life or AD&D plans, even though supported by you.
NOTE: Dependent children may be covered until the end of the calendar year they reach age 19 for Health, Dental, Employee-Paid Life and AD&D. Coverage can be extended until the end of the calendar year they reach age 23, if a full-time student at an accredited college or university.

I am moving out of the area. Can I change my dental carrier?
If you are covered under Aetna, you may change to Delta Dental Plan of Michigan when moving out of the service area.
My dependent is graduating today and will no longer be dependent. When does s/he need to be removed from my health and dental policies?
Children over age 19 (age 23 if attending an accredited college or university) need to be deleted from the dental plan as soon as they are no longer considered dependent. COBRA information for continued coverage on a self-purchase basis for up to 36 months will be sent to the deleted dependent.
Can I add my grandchild to my dental policy?
Yes, under the following conditions:
- Guardianship: Provide legal/court documents of guardianship within 31 days of such guardianship or during an open enrollment period.
- IRS Dependent: If the birth mother/father and child are your IRS dependents and are living in your household, you may add the grandchild within 31 days of birth or during an open enrollment period. An Affidavit of Dependency required.
- Principal Support: A notarized statement indicating the reason for principal support is required.
What is COBRA and how do I receive information on it? How much will it cost?
Under the Consolidated Omnibus Budget Reconciliation Act of 1986 (COBRA), if an employee and/or their eligible dependents involuntarily lose health and/or dental coverage through MSU (i.e., termination of employment, death of employee or retiree, dependent marriage, termination of the same-sex domestic partnership relationship, divorce/separation, etc.), notification of COBRA rights will be sent by MSU Human Resources Benefits. A completed election form and first month's premium must be returned to the MSU Human Resources Benefits within 60 days from the date of notification.
COST: COBRA premiums are 2% above current health and dental rates paid for active employees. Premiums will be billed by the MSU Benefits office.
NOTE: Persons on Social Security Disability, may be allowed to extend COBRA benefits for a longer period at a higher premium. Contact MSU Human Resources Benefits for details.
I enrolled for dental benefits last month. When will I receive my ID cards?
Aetna: Once you have enrolled for your dental benefits, please contact Aetna at 1-877-238-6200 to select your primary care dentist. You must choose a primary care dentist before an ID card will be mailed to you. An ID card will be mailed to you approximately 30 days after you sign up for your dental benefits.
Delta Dental Plan of Michigan: There is no identification card for dental coverage.
